Request for Cooperative Sponsorship Form If you are a member of the Philadelphia Public League you must submit the cooperative sponsorship forms to Dr. James P. Lynch, Executive Director of the Office of Athletics (jplynch@philasd.org) for his approval prior to sending it to District XII. Deadline for Request for submission to District XII approval of Cooperative Sponsorships (for 2024 - 2025) is May 3, 2024. All requests for Cooperative Sponsorships for the 2024-2025 School Year must be property completed and approved by the District Committes and submitted to P.I.A.A. no later than June 1, 2024.
PIAA Medical CIPPE Form (rev. 3-24-24)
This form must be submitted to the appropriate school personnel prior to participation in a sport.
